Oral history is an effective research method to explore and analyse the historical and cultural contexts, interactions, and narratives of those either affected by or responding to HIV/AIDS. However, policymakers and caregivers remain reluctant to consider developing new services or treatments based on oral narratives

    Guidelines for the management of polycystic ovary syndrome (PCOS) focus on lifestyle changes, incorporating exercise. Whilst evidence suggests that aerobic exercise may be beneficial, less is known about the effectiveness of resistance training (RT), which may be more feasible for those that have low fitness levels and/or are unable to tolerate/participate in aerobic exercise. To identify the available evidence on RT in women with PCOS and to summarise findings in the context of a scoping review. Studies utilising pre-post designs to assess the effectiveness of RT in PCOS; all outcomes were included. Four databases (PubMed, CENTRAL, CINAHL and SportDiscus) were searched and supplemented by hand searching of relevant papers/reference lists. Extracted data were presented in tables and qualitatively synthesised. Searches returned 42 papers; of those, 12 papers were included, relating to six studies/trials. Statistical changes were reported for multiple pertinent outcomes relating to metabolic (i.e., glycaemia and fat-free mass) and hormonal (i.e., testosterone and sex hormone-binding globulin) profiles. There is a striking lack of studies in this field and, despite the reported statistical significance for many outcomes, the documented magnitude of changes are small and the quality of the evidence questionable. This highlights an unmet need for rigorously designed/reported and sufficiently powered trials

    Prevalence of dysphagia in people with intellectual disability: a systematic review

    Dysphagia (feeding and swallowing disorder) is associated with serious health complications and psychosocial sequelae. This review summarises international research relating to the prevalence of dysphagia in people with intellectual disability. Studies published from 1990 to July 2016 were identified using Medline, Cinahl, PsycINFO, Web of Science, email requests, and cross-citations. Twenty studies were identified. Dysphagia in people with intellectual disability appears to be associated with more severe intellectual disability, comorbid cerebral palsy, and motor impairments. However, further research with representative samples of people with intellectual disability using adequate methods of assessment are required in order to provide more precise prevalence estimates and clarify factors that may be associated with dysphagia in this population

    British Society of Gastroenterology guidance for management of inflammatory bowel disease during the COVID-19 pandemic.

    The COVID-19 pandemic is putting unprecedented pressures on healthcare systems globally. Early insights have been made possible by rapid sharing of data from China and Italy. In the UK, we have rapidly mobilised inflammatory bowel disease (IBD) centres in order that preparations can be made to protect our patients and the clinical services they rely on. This is a novel coronavirus; much is unknown as to how it will affect people with IBD. We also lack information about the impact of different immunosuppressive medications. To address this uncertainty, the British Society of Gastroenterology (BSG) COVID-19 IBD Working Group has used the best available data and expert opinion to generate a risk grid that groups patients into highest, moderate and lowest risk categories. This grid allows patients to be instructed to follow the UK government's advice for shielding, stringent and standard advice regarding social distancing, respectively. Further considerations are given to service provision, medical and surgical therapy, endoscopy, imaging and clinical trials

    Reducing uncertainty with flood frequency analysis: the contribution of palaeoflood and historical flood information

    Using a combination of stream gauge, historical, and paleoflood records to extend extreme flood records has proven to be useful in improving flood frequency analysis (FFA). The approach has typically been applied in localities with long historical records and/or suitable river settings for paleoflood reconstruction from slack‐water deposits (SWDs). However, many regions around the world have neither extensive historical information nor bedrock gorges suitable for SWDs preservation and paleoflood reconstruction. This study from subtropical Australia demonstrates that confined, semialluvial channels such as macrochannels provide relatively stable boundaries over the 1000–2000 year time period and the preserved SWDs enabled paleoflood reconstruction and their incorporation into FFA. FFA for three sites in subtropical Australia with the integration of historical and paleoflood data using Bayesian Inference methods showed a significant reduction in uncertainty associated with the estimated discharge of a flood quantile. Uncertainty associated with estimated discharge for the 1% Annual Exceedance Probability (AEP) flood is reduced by more than 50%. In addition, sensitivity analysis of possible within‐channel boundary changes shows that FFA is not significantly affected by any associated changes in channel capacity. Therefore, a greater range of channel types may be used for reliable paleoflood reconstruction by evaluating the stability of inset alluvial units, thereby increasing the quantity of temporal data available for FFA. The reduction in uncertainty, particularly in the prediction of the ≀1% AEP design flood, will improve flood risk planning and management in regions with limited temporal flood data

    Addressing the environmental, community and health impacts of resource development: Challenges across scales, sectors and sites

    Work that addresses the cumulative impacts of resource extraction on environment, community, and health is necessarily large in scope. This paper presents experiences from initiating research at this intersection and explores implications for the ambitious, integrative agenda of planetary health. The purpose is to outline origins, design features, and preliminary insights from our intersectoral and international project, based in Canada and titled the “Environment, Community, Health Observatory” (ECHO) Network. With a clear emphasis on rural, remote, and Indigenous communities, environments, and health, the ECHO Network is designed to answer the question: How can an Environment, Community, Health Observatory Network support the integrative tools and processes required to improve understanding and response to the cumulative health impacts of resource development? The Network is informed by four regional cases across Canada where we employ a framework and an approach grounded in observation, “taking notice for action”, and collective learning. Sharing insights from the foundational phase of this five-year project, we reflect on the hidden and obvious challenges of working across scales, sectors, and sites, and the overlap of generative and uncomfortable entanglements associated with health and resource development. Yet, although intersectoral work addressing the cumulative impacts of resource extraction presents uncertainty and unresolved tensions, ultimately we argue that it is worth staying with the trouble
